These are the prescribed forms under the DCS Rules 2007.
Filing a nomination is mandatory for every member and must be completed at the time of admission. It governs the transfer of share and interest in the event of the member’s death.
Step 1: Member fills the Form17 in Triplicate;
Step 2: MC records the nomination in MC Meeting and acknowledges;
